Before submitting a request of a Membership Application Form, please read the information and follow the instructions on the Membership page. Please note that this is only a request for an Application Form and not the actual Application Form. The Membership Application Form will be emailed to you upon receipt of this request. Submission of a completed Application Form is necessary to become a member of the International Association of Free Seventh-day Adventists.
